COMMITMENT
You will commit to 3 large group meetings

 You will get all of your fundraising in by May 22 (one month early)

You will be respectful of the trip leaders at all times and follow their instruction.

What will we be doing?

We will spend 5 days assisting in numerous capacities on the Navajo Nation Reservation in Arizona. We will be hleping rebuild churches, other buildings, and many other physical labor activities. We will also be leading a Vacation Bible School program for the kids each of the days we are there. 

This is an incredible opportunity to join many ALC students on another mission trip, or possibly your first. We are called to take the good news of Jesus all around the world, and also to serve the poor and needy. This WILL BE the highlight of your entire summer. Come with us! 


*We will do some hiking and carrying materials in pretty strenuous terrain, and it will be hot, but not too hot, it's at 7500 ft elevation!

I acknowledge that my child’s participation in Authentic Life’s activities are voluntary and may require involvement in activities that require traveling or physical exertion. Such activities may include, but are not limited to, camps, athletic games, excursions, missions’ trips and meetings. We acknowledge that our child’s participation in any ALC activity presents risk that my child may suffer property damage, bodily injury, or death.


Therefore, in consideration of my child’s participation in the ALC Ministry program activities, we agree to the following: (please initial)

ALC is not responsible for personal belongings.

Students are required to wear seat belts & keep them on while en-route riding in church provided transportation other than rental buses.       

Inappropriate conduct will result in transportation home at parents’ expense

I hereby take the following action for my child, myself, my executors, administrators, heirs, next of kin, successors and assigns: A) I waive, release, and Discharge from any and all claims or liabilities for death or personal injury damages of any kind, which arise out of or relate to my child’s participation in ALC Ministry activities, the following person or entities: ALCC, it’s Senior Pastor, Associate Pastors, Program Staff, Elders, Employees, volunteers, representatives, subcontractors and agents of any of the above. B) I agree not to sue any of the persons or entities mentioned above for any of the claims or liabilities that I have waived, released or discharged herein; and C) I Indemnify and hold Harmless the person or entities mentioned above from any claims made or liabilities assessed against them as a result of my child’s actions. I hereby assume all risk of my child participating in all ALC’s Ministry Activities.


I hereby authorize any licensed physician, emergency medical technician, hospital or other medical or health care facility to treat the minor named herein for the purpose of attempting to treat or relieve any injury received by said minor. I authorize any such Medical Provider to perform all procedures deemed medically advisable in attempting to treat or relieve any such injuries. I consent to the administration of anesthesia as deemed advisable. I realize and appreciate that there is a possibility of complications and unforeseen consequences in any medical treatment, and I assume any such risk for and on behalf of myself and said minor. I further agree to pay all charges for the dental, medical, or hospital care or treatment.


Rules of Behavior expected from each student:

Failure to comply with these expectations could result in your child being sent home at your expense.


1.Particiaption with the group 2. No guys in girls’ sleeping quarters (vice versa). 3. Respect one another, staff and adult leaders. 4. Respect property. 5. Respect and comply with event schedules. 6. No students permitted to drive for events without special parental permission. 7. No offensive or immodest clothing. 8. No fighting, weapons, fireworks, explosives or lighters permitted. 


My child has permission to attend all church sponsored activities. Note: If you desire to limit your child’s participation in any event, please put your restrictions in writing to ALC in advance of that event.
